Damstra Forms lets you use your existing forms - in an app instead of on paper.

The behavior and appearance of the form during data entry in the app needs to be specified using the Template Editor. A template is a blueprint that shows which fields are in the form, and which types of data can be entered, so that data-entry matches the content of your forms and is as easy as possible for users on site.
